A Sharp Decline in Healthcare Coverage

Enrollments for healthcare coverage under the Affordable Care Act declined by more than 2.5 million people in February compared to the previous year. According to new federal data, this steep drop captures individuals who were retroactively removed from coverage after their nonpayment grace periods ended. Health analysts attribute this significant decline primarily to the recent expiration of enhanced premium tax credits. Consequently, many Americans saw their monthly health insurance fees double or triple, forcing them to forgo coverage entirely. This affordability crisis has become a central issue for voters as the upcoming November elections quickly approach.

Hardest Hit States and Economic Factors

A detailed analysis of the data reveals that Ohio and Oklahoma suffered the most significant losses. Both states experienced a massive decline of more than 32 percent in their ACA enrollments over the past year. Following closely behind, states like Arizona, South Carolina, Indiana, and Michigan lost more than a fourth of their covered populations. Meanwhile, Florida still maintains the highest number of residents in the marketplace, but it also recorded the highest net loss by dropping around 443,000 enrollees. Although federal officials suggested a crackdown on fraudulent enrollments played a role, local experts insist that the primary factor was the severe lack of affordability.

The New Mexico Exception and State Exchanges

The data clearly shows that states using the federal marketplace lost larger shares of enrollees than those operating their own state-based exchanges. This difference occurred because several independent states took proactive steps to offset costs for their residents when the federal subsidies expired in January. New Mexico serves as the most extreme example of this successful local intervention. The state actually gained 14 percent more enrollees by using its own funds to replace the lost federal subsidies through mid-2027. Ultimately, local officials emphasized that health insurance should protect people against medical debt rather than actively cause it.
